cpu: Turn cpu_unassigned_access() into a CPUState hook
Use it for all targets, but be careful not to pass invalid CPUState.cpu_single_env can be NULL, e.g. on Xen.

Rename target_phys_addr_t to hwaddr

target-alpha: Move memory helpers to mem_helper.c.
This completes the transition away from AREG0. This patch mustbe last because it requires CONFIG_TCG_PASS_AREG0 set too.
