Rename target_phys_addr_t to hwaddr

target_phys_addr_t is unwieldly, violates the C standard (_t suffixes are
reserved) and its purpose doesn't match the name (most target_phys_addr_t

Use uintptr_t for various op related functions

Use uintptr_t instead of void * or unsigned long in
several op related functions, env->mem_io_pc and
GETPC macro.

target-alpha: Move memory helpers to mem_helper.c.

This completes the transition away from AREG0. This patch must
be last because it requires CONFIG_TCG_PASS_AREG0 set too.
