prep: Kill get_system_io() usage

While ISA address space in prep machine is currently the one returned
by get_system_io(), this depends of the implementation of i82378/raven
devices, and this may not be the case forever.

Use the right ISA address space when adding some more ports to it....

hw: Clean up bogus default boot order

We set default boot order "cad" in every single machine definition
except "pseries" and "moxiesim", even though very few boards actually
care for boot order, and "cad" makes sense for even fewer.

cpu: Make first_cpu and next_cpu CPUState

Move next_cpu from CPU_COMMON to CPUState.
Move first_cpu variable to qom/cpu.h.

gdbstub needs to use CPUState::env_ptr for now.
cpu_copy() no longer needs to save and restore cpu_next.

prep: Make System I/O port 0092 read/write

prep: Add ELF support for -bios

This prepares for switching from OpenHack'Ware to OpenBIOS.

While touching the error handling code, switch from aborting hw_error()
to fprintf()+exit() and suppress failing without -bios for qtest.

hw: move headers to include/

Many of these should be cleaned up with proper qdev-/QOM-ification.
Right now there are many catch-all headers in include/hw/ARCH depending
on cpu.h, and this makes it necessary to compile these files per-target.
However, fixing this does not belong in these patches....
