Fix disk_type error in hypervisor parameter documentation

According to the code, presenting disks as paravirtual is supported on
both HVM and KVM, while IDE works only on KVM. This patch updates docs
to be accurate.

Add support for blktap2 file-driver

Newer Xen versions use blktap2 instead of blktap. This patch adds support
for it in Ganeti.

Fixes Issue 638.

Add documentation for diskstats collector

The data collector for /proc/diskstats was undocumented. Add an entry for it in
the mon-collector man page.

Also, remove some end of line whitespaces from the file.

Fix incorrect manpage reference to htools

the manpages for hcheck, hinfo and hroller have an incorrect reference
to htools(7) which does not exist. This patch links them to htools(1).

This bug also fixes Debian BTS#721824 (http://bugs.debian.org/721824)...
