pithos: Remove 'create' flag from the mapper

The file access mode is now chosen from the file mapper or blocker
according to the function that is being used, i.e., read or write
an object.

New flag 'oflag' determines the mode in which the file has to be

pithos: Add missing blocksize attribute to blocker.

Remove automatic RADOS pool creation.
