arm/xilinx_zynq: Always instantiate the GEMs

Don't conditionalise GEM instantiation on networking attachments. The
device should always be present even if not attached to a network.

This allows for probing of the device by expectant guests (such as
OS's). This is needed because sysbus (or AXI in Xilinx's real hw case)...

arm/xilinx_zynq: Implement CBAR initialisation

Fix the CBAR initialisation by using the newly defined static property.
Zynq will now correctly init the CBAR to the SCU base address.

Needed to boot Linux on the xilinx_zynq machine model.

hw: Clean up bogus default boot order

We set default boot order "cad" in every single machine definition
except "pseries" and "moxiesim", even though very few boards actually
care for boot order, and "cad" makes sense for even fewer.

Drop the now-deprecated arm_pic_init_cpu() in favour of directly
getting the IRQ line from the ARMCPU object.

cpu: Make first_cpu and next_cpu CPUState

Move next_cpu from CPU_COMMON to CPUState.
Move first_cpu variable to qom/cpu.h.

gdbstub needs to use CPUState::env_ptr for now.
cpu_copy() no longer needs to save and restore cpu_next.

xilinx_spips: seperate SPI and QSPI as two classes

Make SPI and QSPI different classes. QSPIPS is setup as a child of SPIPS.
Only QSPI has the LQSPI functionality, so move all that to the child class.

arm: fix location of some include files

The recent rearrangement of include files had some minor errors:
devices.h is not ARM specific and should not be in arm/
arm.h should be in arm/

Move these two headers to correct this.

hw: move headers to include/

Many of these should be cleaned up with proper qdev-/QOM-ification.
Right now there are many catch-all headers in include/hw/ARCH depending
on cpu.h, and this makes it necessary to compile these files per-target.
However, fixing this does not belong in these patches....

xilinx_zynq: Cleanup ssi_create_slave

With the recent m25p80 cleanup there is no need to use
ssi_create_slave_no_init() anymore. Just use ssi_create_slave().
